Massage Intake forms will be sent to the email you provide upon scheduling.

The forms include Health History, Cancellation Policy, Consent to Treatment, and Payment policies. Please have these forms filled out before your first appointment.

If the client is under 18 years of age we ask that the parent or legal guardian contact the clinic before using the online scheduler or filling out forms.

Parking and Location

I am currently practicing out of Intercession Chiropractic in Factoria. When locating the office building look for the large red “Factoria Dentistry” sign and then follow signs for Intercession Chiropractic at the back of the building in Suite 2. Please Park on the side of the building.
